You will assist in reviewing complaints made by the residents, assist residents with information concerning their rights, living wills, etc.
The Social Services Designee will make routine visits to residents and maintain contact with resident's family, involving them with non-medical progress reports.
You will assist in providing solutions for social and practical environmental problems including seeking financial assistance, discharge planning (including collaboration with community agencies), and referrals to other community agencies when specialized assistance is required.
Estimated Salary: $20 to $28 per hour based on qualifications.
